is in no way sponsored, endorsed or administered by, or associated with,
Facebook, Twitter or any other social media platform. You understand that
you are providing your information to the Child Mind Institute. BY ENTERING
THIS CONTEST, YOU AGREE TO THE TERMS AND CONDITIONS LISTED BELOW.

ELIGIBILITY:
This contest is open only to legal residents of the 50 United States and
the District of Columbia who are 18 years of age or older as of the date
the contest begins. Employees and immediate family members of employees of
the Child Mind Institute and its affiliates are not eligible to
participate.

ENTRY PERIOD:
The contest begins November 29, 2017 at 12:00 PM EST and ends January 1,
2018 at 5:00 PM EST. All entries must be received within this time period
in order to be considered for prizes. The Child Mind Institute reserves the
right to modify the deadline for submissions due to circumstances beyond
its control.

ENTRY INSTRUCTIONS:
No offline entries. One entry per person. ALL ENTRIES BECOME THE PROPERTY
OF THE CHILD MIND INSTITUTE AND MAY BE DISPLAYED ON PROMOTIONAL MATERIALS
IN ANY MEDIA THROUGHOUT THE WORLD IN PERPETUITY IN ITS DISCRETION WITHOUT
FURTHER PAYMENT OR CONSIDERATION.

PRIZE:
Three (3) winners will be selected at random by the Child
Mind Institute on December 4, December 11, December 18, December 25, and
January 1. Each winner will receive a $25 Bloomingdale’s gift card and a
2017 Bloomingdale’s Little Brown Bear by Gund. The approximate total retail
value of the prize is $45. Prize includes shipment to the winner’s address
in the United States. Taxes are the responsibility of the winner. ALL
PRIZES ARE AWARED “AS-IS” WITHOUT WARRANTY OF ANY KIND. The Child Mind
Institute reserves the right to substitute a prize of equal or greater
value in the event the offered prize is unavailable.


PUBLICITY:
Except where prohibited, participation in the contest constitutes the
winner’s consent to the Child Mind Institute’s use of the winner’s name,
image, voice, opinions, biographical information, hometown and state for
the Child Mind Institute’s purposes in perpetuity without further payment
or consideration.


COMPLIANCE WITH LAW:
This contest is subject to all applicable federal, state and local laws and
regulations. As an entrant in this contest, it is your responsibility to
determine that you are eligible to enter and to comply with the contest
rules set forth herein. Any violation of the contest rules may result in
your disqualification, in the sole discretion of the Child Mind Institute.

FORCE MAJEURE:
In the event that the Child Mind Institute cannot complete the contest as
planned, including but not limited to by reason of force majeure,
technical failures of any sort or other causes beyond the control of the
Child Mind Institute that corrupt or affect the administration, security,
fairness, integrity or proper conduct of the contest, the Child Mind
Institute reserves the right to cancel, terminate, modify or suspend the
contest.

LIMITATION OF LIABILITY:
By entering this contest, you agree to release, and to defend, indemnify
and hold harmless, Child Mind Institute, Inc., Child Mind Medical Practice,
PLLC and their officers, directors, members, agents, attorneys, employees
and representatives, from any and all liability for any claims, demands,
causes of action, damages, judgments or settlements, or for any losses or
injury to earnings, profits, or goodwill, or for any direct, indirect,
special, punitive, incidental or consequential damages, including
attorneys’ fees and court costs, other than entrant’s out-of-pocket
expenses (if any) associated with entering the contest, arising out of or
relating to this contest, or in connection with any prize won or any misuse
or malfunction of any prize awarded.

GOVERNING LAW:
This agreement shall be governed by and construed in accordance with the
laws of the State of New York.
