Nomination Form

Each year, college scholarships are awarded to high school students who show exceptional promise in the fields of mental health research, neuroscience or biomedical engineering. Candidates are nominated in their junior year of high school and winners are celebrated in their senior year.

 
Nominations may only be submitted by school faculty. A copy of the student’s transcript is required to be submitted with the nomination form. Once a nomination has been received, the student will be asked to complete a brief form to collect background information and share at least one work sample.  This request will be sent to the student email provided in the nomination form.   • 1. Describe the nominee's record of academic achievement to date in high school science courses. No more than 300 words, please.
  • 2. Describe the nominee’s involvement in science or research-related extracurricular activities including participation in competitions, clubs, fairs or programs as well as volunteer work and internships. No more than 300 words, please.
  • 3. Describe how the nominee demonstrates an interest in and capacity for innovation within the field of science as evidenced by the pursuit of or creation of new knowledge that is not part of the standard curriculum. Please remark on any research activities that the candidate has participated in or papers/ presentations/publications that the candidate has made a contribution to or independent research that the candidate has initiated.
