Nomination Form
Each year, college scholarships are awarded to high school students who show exceptional promise in the fields of mental health research, neuroscience or biomedical engineering. Candidates are nominated in their junior year of high school and winners are celebrated in their senior year.
Nominations may only be submitted by school faculty. A copy of the student’s transcript is required to be submitted with the nomination form. Once a nomination has been received, the student will be asked to complete a brief form to collect background information and share at least one work sample. This request will be sent to the student email provided in the nomination form. See guidelines and FAQ for more information.
