Thank you for filling out the application for the Child Mind Institute Summer Program. Once you’ve completed the form, please submit it using the button below. Once your application is reviewed and deemed appropriate, the next step will be to schedule a screener with a clinician on the Summer Program team. The screener fee is $200, which covers the clinician’s time for reviewing the application and meeting with the child and parent(s).
